USTP COLLEGE ADMISSION APPLICATION

IMPORTANT:

The Application Form shall be accomplished electronically. Your photo is required in this form. Upload your 2×2 ID Photo in JPEG file with file size not more than 200 kilobytes.
Please be advised that only those who have accomplished the e-Application Form shall be processed for the admission test schedule.

Application Procedure for College Freshmen S.Y. 2019-2020

1. Carefully review all the fields in the e-Application Form and provide complete and accurate information. Click the Apply Now button.

2. If you have successfully submitted the e-Application Form, submit the following requirements to the Admissions and Scholarships Office at the ICT Bldg. Room 106.

For Graduating Grade 12 Applicants

a. Photocopy of Form 138/Report Card or Form 137. Electronic copy of grades must be signed by the Adviser or High School Principal.

b. Photocopy of Grade 12 School ID.

c. 2 pcs. 2×2 recent identical ID photo with white background (selfie photo not accepted)

d. Additional Requirements if Applicable:

c.1 One (1) Photocopy of 4Ps Beneficiary Certification from Department of Social Welfare and Development

c.2 One (1) Photocopy IP Certification from National Commission on Indigenous People (NCIP) or a legitimate Tribal Chieftain.

c.3 One (1) Photocopy of PWD Certificate from the City Social Welfare Department (CSWD)

3. Secure the registration slip from the Admission staff and note the venue and schedule of your admission test.

For High School Graduate of the old Basic Education Curriculum

a. Photocopy of Form 138/Report Card or Form 137

b. 2 pcs. 2×2 recent identical ID photo with white background (selfie photo not accepted)

c. Additional Requirements if Applicable:

c.1 One (1) Photocopy of 4Ps Beneficiary Certification from Department of Social Welfare and Development

c.2 One (1) Photocopy IP Certification from National Commission on Indigenous People (NCIP) or a legitimate Tribal Chieftain.

c.3 One (1) Photocopy of PWD Certificate from the City Social Welfare Department (CSWD)

3. Secure the registration slip from the Admission staff and note the venue and schedule of your admission test.

4. Report on the scheduled date and venue indicated in your registration slip. Applicants without the registration slip will not be allowed to take the admission test.

5. Check for the list of passers on the date of release of admission test results. Visit the official university website, Facebook page or check the list which shall be posted outside the Admissions and Scholarships Office.

Application Procedure for Transfer Students and Second Courser

1. Applicants must present an original copy and photocopy of Transcript of Records and Honorable Dismissal to the concerned Program Chairman for evaluation. Evaluation slip approved by the college Dean shall be issued to the applicant who is found qualified to take the admission test. Second courser applicants are required to pay for the Admission Test. Secure the Admission Test Payment slip from the Program chair and pay P450.00 to the Cashier’s office.

2. Carefully review all the fields in the e-Application Form and provide complete and accurate information. Click the Apply Now button.

3. If you have successfully submitted the e-Application Form, submit the following requirements to the Admissions and Scholarships Office at the ICT Bldg. Room 106.

a. One (1) Photocopy of Transcript of Records and Honourable Dismissal

d. For Second Courser applicants only, present the Official Receipt of Payment

e. Additional Requirements if Applicable:
