Rules and Regulations

  1. Priority in the use of the AV Center is given to instructional-related activities requiring of its resources for one class period. Extension shall be considered on a case-to-case basis.
  2. Activities such as musical shows, stage play/theater presentations, general assemblies, and student organization recruitment orientation shall be entertained in the AV Room with special concurrence with the Audiovisual Services in-charge.
  3. The party in-charge of the activity is expected to be personally present at all times. He/She shall be responsible for the after-care and the strict implementation of the "NO SMOKING, EATING AND DRINKING" policy inside the AV Room.
  4. Installation of additional power connection and fixture should be referred first to the AVC in-charge. Loaning of equipment and materials from the AV Center is not allowed.
  5. Reservation is on a FIRST-COME-FIRST-SERVE basis. The AVC management reserves the right to cancel or revoke confirmed reservations in case of misrepresentation and/or violation of the policies, rules and regulations.
  6. Any damage incurred during the use of any group shall be charged against the person applying for reservation and the person endorsing the approval of the application is also jointly and severally liable.
  7. Any complaint should be directed in writing to the University President copy furnished to the Director of Library and Audiovisual Services and the AVC in-charge.

NOTE:

A reservation entered in the log book is considered tentative until the reservation form is accomplished and returned to the AVC Office.

The reserving group is responsible for the return of the AVC form to avoid cancellation of the reservation.


STEPS IN RESERVING THE USE OF THE AV ROOM

A. Classes

  1. For class activity needing AV resources such as videotape, slide projector or overhead transparencies where the use of the regular class room is not possible, the AV Room may be reserved.
  2. Check with the AV Assistant regarding the availability of the facility. Make a tentative reservation.
  3. Get and fill out the AVC form from the AVC In-Charge at the Engineering/Graduate School Library indicating the equipment and materials for a tentative reservation or download it here.
  4. Have the form endorsed/signed by the unit/department head or class instructor.
  5. Cancellation of the reservation should be relayed to the AVC before the scheduled activity.

B. Student Organizations

  1. For extra-curricular or co-curricular legitimate student organizations, the use of the AV Room must be limited to instructional-related activities such as seminars, forums and video showings. Concerts, musicals and stage presentations, general assemblies, recruitment for student organizations and prayer meetings are not allowed.
  2. Check with the AV Assistant regarding the availability of the facility. Make a tentative reservation.
  3. Write a formal letter of request endorsed by the Moderator and the Vice-President of Student Affairs to the AVC In-Charge indicating the activity, the name of the speaker/s (if any), the AV equipment/materials needed, and the date and time of actual use. If the activity requires registration fees from the participants or if it is an income-generating activity, the organization is required to pay a fee of P500.00/hr directly to the Finance Office for the use of the facility. For activities scheduled on weekends, an overtime fee/honorarium shall be given to the AV Assistant by the student organization.
  4. Get and fill out the form from the AVC In-Charge at the Graduate School Library or download it here. Indicate the activity, the date and time of actual use and the equipment and materials needed. Attach the letter of request.
  5. Return the AVC form for final confirmation by the AVC In-Charge.
  6. The form cannot be confirmed unless the P500.00/hr fee is fully paid by the student organization (if the activity is an income-generating activity).

C. Academic Departments and Units

  1. The AV Room may be used by any academic unit or department for projection purposes, meetings, seminars, fora or conventions where other rooms or facilities are not possible.
  2. Check the availability of the AV Room for tentative reservation indicating the time and date of actual use.
  3. Get and fill out the form from the AVC indicating the activity, date and time of actual use including the materials and equipment to be used for necessary preparation. The reservation shall be endorsed by the Vice-President for Academic Affairs for academic units and by the Vice-President for Administration for non-academic units.
  4. Return the AVC form for final confirmation by the AVC In-Charge.

D. Outside Group

  1. A letter requesting the use of the AV Room addressed to the President of Mindanao University of Science and Technology must be presented.
  2. Make a tentative reservation with the AVC indicating the date and time of actual use and activity. Drama and musical presentations, concerts, stage plays, prayer meetings, recollections and the like are not allowed in the facility.
  3. Get and fill out the AVC form from the office indicating the date and time of actual use, activity and the person endorsing the groups' activity.
  4. A fee of P500.00/hr rental and cost of energy consumption and maintenance cost for the AV Room (inclusive of sound system only) must be paid at the cashier's office before the reservation is confirmed. There is an additional fee of P50.00/hr representing the honorarium/overtime fee of the AVC staff who will assist all throughout the activity during weekends/holidays. Other fees for the use of the AV equipment (if needed ) is additionally charged.
  5. Return the form to the AVC for the confirmation, attaching the copy of the receipt of the payment and the letter approving the use of the facility from the President of the University. If the group is not required to pay, this should be explicitly indicated by the authority of the President.
